Guide to filling the W-9 form (US)
W-9 form is an Internal Revenue Service (IRS) tax form used to confirm a person's name, address, and taxpayer identification number (TIN) for employment or other income-generating purposes. The confirmation can be requested for either an individual defined as a U.S. citizen or a person defined as a resident alien.
The information on the W-9 form is used by the payer to accurately report payments made to the IRS on Form 1099 at year-end. The W-9 form is primarily a tool to collect the required tax information from recipients of payments, and it does not get sent directly to the IRS.
Guide to fill the W-9 form
Enter details as they appear on your income tax return.
For Individuals, enter the following information
- Name: Legal name as shown on your income tax return.
- Business Name/Disregarded Entity Name: Leave empty if you don’t have a trade, business, or a disregarded entity name.
- Check box for federal tax classification
- Exemptions: Refer to exemption codes on the W-9 form (pages 3-4).
- Address: Include building number and street name for Address line 1. Include apartment, suite, unit, building, or floor number for Address line 2. Note that the address has to match your income tax return.
- Tax Identification number (TIN): For individuals, the Social Security Number (SSN) is equivalent to TIN.
- Sign form: Signature should contain full legal name.
